This program provides guidance and support that many young people in Greene County need to
transition into becoming an independent individual.

HT-ILT is a program designed to help youth and young adults aged 16-25 living in Greene Co. that are experiencing serious emotional disturbances, serious mental illnesses, co-occurring disorders, and/or intellectual disabilities.

 

Our goals:
Introduce our program, educate potential clients and offer information to the community so that they are able to refer youth and young adults to the program.
To reduce stigma among youth and community regarding mental health and treatment.

We offer a variety of services provided by certified, licensed and experienced staff along with the Healthy Transitions Program that include:

Linkage to Medication Management
Employment/Education Support to help obtain a G.E.D, H.S. Diploma, College applications, job applications, resume support, etc. (application fees are covered by HT)
Peer Support
Case Management
Therapy sessions

We hold a peer group every Tuesday from 2pm-3pm and every Thursday from 2pm-5pm. During these groups we learn how to:

Prepare healthy meals, cook, and wash dishes
How to do laundry
Cleaning
Basic Hygiene
Exercise techniques
Money management and budgeting
Socialization
Obtaining a drivers license
Managing Anxiety
Coping Skills
And much more!
